ELIGIBILITY REQUIREMENTS
ELIGIBLE DEPENDENTS
Your eligible Dependents may also participate in the Plan. An eligible Dependent is considered to be:
- your Spouse
- your or your Spouse’s child who is under age 26, including a natural child, stepchild, a legally adopted child, a child placed for adoption or a child for whom you or your Spouse are the legal guardian; or
- an unmarried child age 26 or over who is or becomes disabled and is your legal dependent.
To be eligible for coverage under the Plan, a Dependent must reside within the United States.
Note: Your Dependents may not enroll in the Plan unless you are also enrolled. If you and your Spouse are both covered under the Company’s Benefit Plan, you may each be enrolled as a Participant or be covered as a Dependent of the other person, but not both.

Making Changes During the Year
The choices you make during Open Enrollment remain in effect for the entire year (January 1 – December 31). In order to make changes to your healthcare elections during the year, you must have a qualified family status change. Qualified family status changes allow you to add or drop dependents or change your level of coverage. These status changes include:
- Marriage, divorce or legal separation
- Gain or loss of an eligible dependent for reasons such as birth, adoption, court order, disability, death, marriage, or reaching the dependent child age limit
- Changes in your spouse’s employment resulting in loss or gain of coverage
- Changes in your spouse’s benefit coverage with another employer that affects benefit eligibility
The change to your benefit election(s) must be consistent with your change in status and elected within 30 days of the Life Event.
